Are you a compassionate and dedicated nurse seeking a rewarding career? The Converse Home, a warm and welcoming assisted living community in Burlington, VT, is seeking a per diem nurse to cover open shifts across both our memory care and traditional communities.

  Key Responsibilities:

• Provide high-quality nursing care with compassion and professionalism.
• Administer medications and treatments in accordance with care plans and physician orders.
• Monitor residents’ health status and promptly report changes to the healthcare team.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Educate and support residents and their families.
• Maintain accurate, timely, and thorough medical documentation.

 
Qualifications:

• Must have a Current RN/LPN license in the state of Vermont.
• Experience in assisted living, long-term care, or a related setting preferred.
• Strong organizational and time-management skills.
• Excellent communication and interpersonal skills.
• A team-oriented, patient-centered, and compassionate approach to care.

We have beautifully landscaped grounds that include large flower and vegetable gardens, a gazebo and many areas for our residents to enjoy being outside.
