Overview
Social Worker – Medical Social Worker travel assignment in Cumming, GA. Start date: 07/13/2026 for 13 weeks. Weekly pay range: $1,679–$1,790 per week, with guaranteed 40 hours. Shift: 12:00 PM–8:30 PM, Monday–Friday. Setting: hospital-based, travel assignment.

Responsibilities
- Conduct psychosocial assessments of patients and families to identify barriers to care, discharge needs, and psychosocial risk.
- Coordinate patient care with the interdisciplinary team to support timely disposition and safe transitions.
- Provide counseling and linkage to resources for coping with illness, treatment decisions, and family communication.
- Develop and implement comprehensive care plans; document in the electronic medical record; ensure compliance with policies and regulations.
- Participate in care conferences and discharge planning meetings; advocate for patient-centered planning.

Requirements
- Active Georgia LCSW license
- AHA BLS certification
- Minimum 2 years of recent hospital-based medical social work experience
- Ability to work in a hospital-based setting in Georgia
