We’re seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused—no prescribing, no treatment plans.

What You’ll Do
Conduct comprehensive in-home wellness assessments
Educate members on preventive care benefits
Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations
Complete documentation using provided EMR tools
Visits are face-to-face and take place where members live—allowing for more personalized, impactful care.
Requirements
Active State NP license and Board Certification
Valid NPI
Minimum 3 months of wellness exam experience preferred
Comfort with EMR/documentation systems
Ability to travel
